Immunization

IMMUNIZATION INFORMATION:  ACTION MAY BE REQUIRED


During the current COVID-19 situation, the U.S. CDC and Maine Immunization Program are recommending that children continue to receive their routine vaccinations to prevent the spread of vaccine-preventable disease.  


Parents/Guardians are required to submit documentation of immunization by the first day of school, September 8th, 2020, to attend school.

The school health record must include the following information:

  • 5 DTaP (Diphtheria, Pertussis, Tetanus). 4 DTaP if 4th is given on or after 4th birthday)

  • 4 Polio (if 4th dose given before the 4th birthday, an additional age appropriate vaccine should be given on or after the 4th birthday)

  • 2 MMR (measles, mumps, rubella)

  • 1 Varicella (chickenpox) or reliable history of disease from provider

Entering GRADE 7:  Tdap and Meningococcal vaccination (MCV4) - One dose of each is required for entry into 7th grade. If you’re not sure if your child has had these vaccinations please contact your child’s provider to inquire/schedule an appointment.

Immunization Exemptions (applies to all students not meeting immunization requirements):

  • Philosophical or Religious - Parents/Guardians are required to submit a signed Exemption Form each year by the first day of school, September 8th to attend school (Exemption Form attached below).

  • Medical - Parents/Guardians are required to submit a letter each year from their child's health care provider (physician, nurse practitioner, or physician's assistant) by the first day of school, September 8th, to attend school.

PLEASE NOTE:  Beginning in September 2021, ONLY Medical Exemptions will be allowed per the amended Maine Immunization Law LD 798 https://legislature.maine.gov/legis/bills/bills_129th/billtexts/HP058601.asp

MSAD #28 POLICY IMMUNIZATIONS OF STUDENTS

All students who enroll in the school district are required by Maine law to present a certificate of immunization or evidence of immunization or immunity against poliomyelitis, diphtheria, pertussis (whooping cough), tetanus, measles, mumps, rubella and varicella (chicken pox) and meningococcal meningitis.

Non-immunized students shall not be permitted to attend school unless one of the following conditions are met:

  1.  Parents and guardians will provide written assurance that the child will be immunized, followed by proper documentation within ninety (90) days. This option is available only once to each student during their school career; or

  2.  The parents/guardians provide a physician’s written statement each year that immunizations against one or more diseases may be medically inadvisable (as defined by law/regulation); or

  3.  The parents/guardians state in writing each year that immunization is contrary to their sincere religious or philosophical beliefs.

The Superintendent shall exclude from school and school activities any non-immunized student when there is a danger to the health of others as provided by law.

The Superintendent/designee is directed to develop such administrative procedures as are necessary to carry out this policy and comply with statutory requirements.
